Service Description
This is for patient's already seen by Optimum looking for medical advice for a simple issue where the provider will review the chief complaint, medical history and develop a plan with the patient. This may or may not include the ordering of prescriptions if needed and/or labs. All will depend on the situation. This service is for simple issues that the patient would like to consult with a qualified medical provider for issues such as: -Common cold symptoms -Mild pains or discomforts -Rx refills for patient's with temporary inaccessibility to their primary or specialists. Some medications may not qualify. Please indicate the medication (s) and conditions they are treating when booking. If unable to prescribe the appt will be cancelled and you will be refunded in full. If you think you or your loved one is having a medical emergency, call 911 immediately This visit is not for the following issues: -Severe Abdominal pain -Profuse vomiting -Chest Pain -Shortness of breath -Severe pain, dizziness
Cancellation Policy
To cancel or reschedule please call us at least 24 hours in advance to avoid forfeiting the deposit. We will attempt to make accommodations for re-bookings as much as possible.
