Optimum This service is for simple issues that the patient would like to consult with a qualified medical provider at their home for issues such as: -Common cold symptoms -Mild pains or discomforts -Rx refills for patient's with temporary inaccessibility to their primary or specialists. Some medications may not qualify. Please indicate the medication (s) and conditions they are treating when booking. If unable to prescribe the appt will be cancelled and you will be refunded in full. At this visit Optimum Wellness & Hydrations will 1. Interview the patient and review medical complaints 2. Complete a physical assessment to include blood pressure, temperature and pulse oximetry 3. Assess Medical History 4. Formulate a possible diagnosis of minor complaints 5. Make recommendations based on 6. Recommendations may include -Make contact with PCP or specialist for continuation of established care -Refer to a higher level of care such as urgent care or ER, if applicable -Order labs or tests that can be obtained on the same visit, labs sold separately. -Make recommendations based on findings -Send prescriptions if needed, to the local pharmacy If you think you or your loved one is having a medical emergency, call 911 immediately This visit is not for the following issues: -Severe Abdominal pain -Profuse vomiting -Chest Pain -Shortness of breath -Severe pain, dizziness
